North East Hampshire and Farnham Making Connections is an NHS-funded project, which provides a “social prescription” service for adults aged 18+, offering person-centred support to help them achieve their health and wellbeing outcomes.
This project is being managed by Hart Voluntary Action in the North East Hampshire area and Age UK Surrey in the Farnham area. The Making Connections Co-ordinator being recruited will be employed and managed by Hart Voluntary Action, and join an existing project team.
The Making Connections Co-ordinator’s role includes the following: providing personalised support to individuals referred into the service, helping them with goal-setting and accessing community-run activities and services; and working with GP surgeries and other health and social care professionals.
We are looking for someone with excellent communications and organisational skills, who is able to work on his/her own initiative and within a team. IT knowledge and some database experience would also be useful. The Co-ordinator will be visiting people in their own homes so access to own transport is essential. Training will be provided. DBS Checks will be required for these posts.
