Recruiting: Home from Hospital Co-ordinator (North East Hampshire)


Home from Hospital Co-ordinator (30 hours per week fixed term contract to 31 May 2020)

Work Locations: Frimley, Surrey and Fleet, North East Hampshire

Monthly salary: £1,587 (30 hours per week)

Fixed-term contract to 31 May 2020

Pension + business travel mileage costs  

30 hours per week on a shift basis over 7 days a week

The Making Connections Home from Hospital service is a pilot service for adults living in North East Hampshire who require low-level support on a short-term basis following a stay in hospital (Frimley Park Hospital, Fleet and Farnham Community Hospitals).   Support provided will include ensuring that clients are settled back in their home environment and have access to appropriate support in the home to remain independent, and assistance with accessing local activities and services in the community to help them maintain their health and wellbeing.

This project is being managed by local charity Hart Voluntary Action, and the Home from Hospital Co-ordinators will join a staff team, who will provide back-office support and guidance and information on local services, which Home from Hospital clients could access.  Where appropriate, Home from Hospital clients may be referred onto the main Making Connections social prescription project for ongoing support.

The Home from Hospital Co-ordinators’ role includes the following: meeting with clients before they are discharged from hospital to agree arrangements for support before and after discharge from hospital (eg assistance with shopping, attending medical appointments, collecting prescriptions, ensuring they have adequate heating/ food in the house, arranging access to telecare services, and, helping them with goal-setting and accessing community-run activities and services); conducting homes visits or follow-up by telephone calls, and working with health and social care professionls.

We are looking for soemone  with excellent communication and organisational skills, who are able to work on their own initiative and within a team.  Prior experience of person-centred planning, good IT skills, including using databases are desirable.  The Co-ordinators will be travelling for work so access to own transport is essential.  Training will be provided.  DBS Checks will be required for these posts.  The availability to work on a shift basis including weekends is also required.
